COVID Vaccines Available Through Select CompuNet Patient Service Centers
As a reminder, CompuNet Clinical Laboratories offers both the Pfizer and Moderna COVID vaccines at four patient service center locations to individuals 12 years of age or older.
Individuals can opt to receive their initial COVID 2-dose vaccine series or the first or second COVID booster. For the most up-to-date guidance on when it is appropriate to receive each of the vaccine doses, please visit the CDC website at: https://www.cdc.gov/.
Appointments may be scheduled at www.premierhealth.com/vaccine. Patients should remember to bring their vaccine card with them to their appointment if they are receiving their second initial dose or a booster shot.

Appointments are recommended; however, vaccine requests will be honored during lab work visits if slots are available.
- CENTERVILLE: Miami Valley Hospital South CompuNet Patient Service Center, 2350 Miami Valley Drive (Thursdays)
- DAYTON: Miami Valley Hospital, One Wyoming, Berry Building CompuNet Patient Service Center (Tuesdays)
- ENGLEWOOD/DAYTON: Miami Valley Hospital North, 9000 N. Main St., CompuNet Patient Service Center, Suite 303 (Wednesdays)
- MIDDLETOWN: Atrium Medical Center, One Medical Center Drive, CompuNet Patient Service Center, 1st floor - inside AMC hospital – (Fridays)
