Appointment/Procedure Preparation
If you feel a life-threatening emergency has occurred, do not wait for an appointment. Please call 911. Appointments can also be made through our secure MyChart portal.
New patients and current patients looking to schedule an appointment for a non-emergency should call our office at (513) 755-1912(513) 755-1912. Our team is excited to be able to offer you exceptional access to care by providing same-day appointments. We believe quality care is best achieved through a consistent physician-patient relationship.
Depending on your specific healthcare needs, you might be asked to do a 12-hour fast prior to your appointment so blood work can be completed.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
At your visit, you will be provided with information about your rights under the Health Insurance Portability and Accountability Act (HIPAA), as required by federal law.
What to Bring to Your Appointment
All patients are asked to bring the following things to each visit:
- Current insurance card
- A photo ID
- All of your current medications, including any supplements and over-the-counter medications
- A list of any allergies
- A list of past surgeries
- A list of other physicians you see and medical issues you see them for
If you do not have your insurance card with you, you will be asked to reschedule.
When you arrive for your appointment, our staff will verify your address, phone numbers and insurance information. If you would prefer, you can also provide our office with your email address, and we can contact you by email in the future.
After Your Visit or Procedure
When your visit is done, you will be given an After Visit Summary, which includes the details of your visit, instructions, educational materials and suggestions. You also will be given the opportunity to sign up for MyChart – an Electronic Medical Record (EMR) – so you can review your medical record from home and communicate with the team at Liberty Family Medicine through the secure Website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. View the MyChart Tip Sheet to see how to request prescription refills.
Our office shares lab and X-ray results promptly by phone, letter or through MyChart.
Liberty Family Medicine refers most patients to Atrium Medical Center (Atrium).
Billing and Insurance
Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network here. Liberty Family Medicine accepts most of these insurance plans.
Co-payments, deductibles and/or past due balances are due at the time of service.
We accept payment by cash, check or credit card (Visa, MasterCard, Discover and American Express). If you are unable to comply with our payment practices, please let us know when you call to make your appointment. Our receptionist will direct you to our accounts receivable representative to discuss payment arrangements.
