We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:

If you need any special instructions for your visit, they will be discussed with you during your initial phone conversation.
What to Expect
If you’re a new patient, please be sure to bring your current medications to your first appointment, as well as a list of allergies.
Both new and established patients need to bring insurance cards to every visit.
You will be asked to fill out a new Patient Registration Form every year to keep your information up to date and renew your permission to bill your insurance.
Both new and established patients who have had imaging done at a location that’s not a Premier Health facility should bring a CD with the images on it to your appointment.
All patients should come to each visit prepared to pay any co-pays, deductibles or co-insurances at the time of your visit.
After Your Visit or Procedure
At the end of your visit, you will receive an After Visit Summary, which will include your physician’s instructions and any appropriate education materials or suggestions.
You will be asked to sign up for MyChart so our team will be able to communicate with you through the secured website, if needed.
Through MyChart, you can also schedule appointments, check your current and past lab results, and ask medical questions.
Our office will communicate lab and X-ray results promptly by either phone, through the mail, or on MyChart. Always feel free to call for information if you believe your results are taking longer than expected.
Billing and Insurance
Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network offices. Premier Blood and Cancer Center accepts most of these insurance plans.
