Premier Family Care of Kettering provides patients throughout the Miami Valley with quality healthcare for the entire family.

Premier Family Care of Kettering’s office is conveniently located in Kettering, so patients throughout the region can easily access the care.

Appointment/Procedure Preparation

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Personal Health History Form
• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

All patients are asked to bring the following things to each visit:

• A list of your known allergies
• Current health insurance card(s)
• Patient registration forms, if you filled them out in advance
• Photo identification
• Your medications or a list of the medications you are currently taking, including any supplements and over-the-counter medications

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

What to Expect

New patients may be asked to complete a health history questionnaire to assist the physician in determining the best course of treatment, and or screening and prevention measures. Premier Family Care of Kettering will strive to make your medical record complete with your personal and family health history, update current medications, record screening tests you’ve completed, such as recommended cancer screenings, vaccinations, etc. This may take more time at your initial visit

Premier Family Care of Kettering will encourage compliance with recommended screening tests, in the interest of maintaining your good health.

After Your Visit or Procedure

At the end of your visit, you will be given an After Visit Summary, which will include your physician’s instructions, and possibly educational materials. You also will be given instructions to sign up for MyChart – so you can review your medical record from home and communicate with Premier Family Care of Kettering through the secure website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions.

Our office shares lab and X-ray results promptly by phone, letter or through MyChart.

Billing and Insurance
