Appointment/Procedure Preparation

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Returning patients should bring their new insurance card(s) when applicable. You may be asked to complete a new Patient Registration Form during your first visit of each new year.
Please come prepared to pay any co-pays, deductibles, or co-insurances at the time of your visit.
After Your Visit or Procedure
After your visit, you will receive instructions for appointments or referrals. If you had tests done, you will receive a follow-up phone call or a letter with your results.
Billing and Insurance
View the list of the major insurance plans accepted by most Premier Physician Network offices. Prmier Health Family Care - North accepts most of these plans.

About Electronic Medical Record (EMR)
The physician and staff will document your medical information, including your list of current medications, immunizations and preventive health screenings, personal health history, family health history, any current active medical problems, vital signs and the details of each visit with the physician, in our electronic medical record. Each exam room is equipped with a computer terminal. While the medical assistant and the physician focus on your visit, they may also enter important information into your medical record, order tests, and list medications and other health information.
How does the Electronic Medical Record (EMR) benefit you?
By entering your medical information into our system—which is accessible only by healthcare providers in the Premier Health network of physicians and hospitals—your doctor makes sure that, should you visit a Premier Health hospital or specialist, that physician has your most accurate and up to date medical information. This is especially important in case you have a medical emergency or illness that requires hospitalization.
Another benefit is that we can quickly get your test results from services performed in a Premier Health hospital or diagnostic facility, and laboratory services performed at Compunet Clinical Laboratories.
Referring you to Premier Health facilities is easy and efficient, as instructions from the physician to the hospital or facility are sent electronically. The facility will contact you within 24 hours to schedule your services and instruct you on test preparation. Most test results are available for the physician to review electronically in 48 to 72 hours; many are available even sooner.
Prescriptions and refills of medications can be sent electronically to your pharmacy, or—in most cases—to your mail order pharmacy service. This speeds the process for you and improves communication and accuracy between the physician and pharmacist.
All healthcare service providers are bound by Premier Health's confidentiality and compliance policies, as well as government regulations regarding protection of your privacy, when using the EMR. Your information is safe and secure, accessible only by those directly involved in your care.
