Appointment/Procedure Preparation
Premier Health Internal Medicine of Englewood is currently accepting new patients. Please contact our office at (937) 832-9322(937) 832-9322 to schedule an appointment.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
If you are a new patient, please be sure to bring your actual medications along with your insurance card(s).
If you are a current patient, please bring any new insurance card(s) when applicable and be prepared to complete a new Patient Registration Form at your first visit of each year.
All co-pays, deductibles, or co-insurances are due at the time of your visit.
After Your Visit or Procedure
Following any tests or procedures done outside of our office, you will receive a telephone call or MyChart message regarding the results within two weeks. Please contact our office if you have not heard from us within two weeks of your test or procedure.
The receptionists at Premier Health Internal Medicine of Englewood work very hard to schedule any tests or procedures done outside of the office during the time of your visit. If your insurance company requires us to obtain a prior authorization before a certain test or procedure can be done, we ask that you allow 24 to 48 hours for us to schedule the appointment. We will contact you with the appointment information.
If your physician orders any further testing, procedures or referrals to a specialist our staff will forward those requests to the central scheduling department or to the specialist office for you. They will contact you directly to schedule a convenient time for you.
