Appointment/Procedure Preparation
To schedule an appointment, call our office at (937) 429-4369(937) 429-4369. New patients can download the following forms or request forms to be emailed or sent via U.S. Mail.
Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
Please complete these forms and bring them with you to your first appointment. You should also review the Notice of Privacy Practices prior to your first visit.
What to Expect
If you are a new patient, please be sure to bring your medications to your appointment as well as a list of allergies and your insurance card(s). Established patients should bring your insurance cards to each visit. You will be asked to complete a new Patient Registration Form annually so that your information is updated and you renew permission to bill your insurance.
All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
After Your Visit or Procedure
At the end of your visit you will receive an After Visit Summary which will include your physician's instructions and appropriate education materials or suggestions. You will be asked to sign up for MyChart so that we will be able to communicate via our secured website, back and forth, as needed, MyChart also allows you to schedule appointments, check your current and past lab results, and ask other medical questions. Our office communicates lab and x-ray results promptly either by phone, letter, or MyChart.
For chronic illnesses, follow-up appointments will be scheduled every 3 months.
