Appointment/Procedure Preparation
Premier Health Rheumatology is currently accepting new patients. To schedule an appointment, please call (937) 223-4900(937) 223-4900.
Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
If you are a new patient, please bring your health insurance card and a list of all medications. Established patients should bring new insurance card(s) when applicable. You may be asked to complete a new Patient Registration Form once a year.
All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.
After Your Visit or Procedure
After your visit, you will receive instructions for making a follow-up appointment. If tests have been ordered, you will receive a follow-up phone call or a letter with your results.
