Medication Therapy Management: Anticoagulation Services

Premier Health partners with providers to offer pharmacist-managed medication therapy management services, including an anticoagulation clinic. Through a referral-based program and a consult agreement with a patient’s primary or specialty provider, the clinical pharmacist will work one-on-one with the mutual patient to safely and successfully manage his or her anticoagulation therapy. Patients will receive comprehensive and individualized care based upon current FDA-approved indications and accepted clinical practice guidelines.

What Services are Provided?

Warfarin Management

  • Point-of-care finger stick tests to measure INRs
  • Dosage adjustments and medication scripts/refills
  • Ongoing education, including interactions with food, medications, supplements and vitamins
  • Established working relationships with local home health facilities post-hospital discharge
  • Continued monitoring while traveling or during transitions of care when interruptions in or bridging of anticoagulation may be warranted due to a recent hospitalization or scheduled procedure

Direct Oral Anticoagulants (DOACs) Management: Eliquis, Pradaxa, Xarelto, Savaysa

  • Dosage adjustments and appropriate therapy evaluations based on a patient’s clinical and financial needs, as well as periodic assessments of renal function and CBC monitoring
  • Ongoing education, including interactions with food, medications, supplements and vitamins
  • Peripheral guidance on managing DOACs surrounding surgeries or other invasive procedures

How to Refer Patients?

Our medication therapy management services are covered by most insurances. To refer a patient, you must have a current consult agreement in place with us; we can work with you to complete the agreement. Orders may then be placed electronically as an EPIC order (searching by “anticoag referral” in database or via Code: CON30488 for warfarin and CON304548 for DOACs) or they can be faxed utilizing the respective clinic’s referral form:

A record of each patient’s visit will be available in EPIC or faxed to you for review per request.
